What Recent Studies are Saying

Several recent studies have lauded the benefits and possibilities of embracing fat transfer to compensate for the effects of breast implant removal. The Journal of the American Society of Plastic Surgeons and a team led by internationally renowned professor of surgery Dietrich Doll have found promising results in clinical studies of this procedure.  

Both studies began with surgeons looking to report on new ways to offer patients alternatives to the negatively perceived consequences of implant removal. For many women, one of the studies stated, “patients [would] rather bear the consequences of severe fibrosis than to have their implants removed.”

What Is Recovery Like?

After surgery, you will most likely experience some bruising near the liposuction area, though this goes away in 7-10 days. 

The rest of the recovery process can vary considerably based on your unique case. Dr. Sajjadian will discuss the full treatment and recovery plan with you during your consultation. However, most patients can assume the process will look something like this general guide: 

  • Two to fourteen days after surgery you can return to work
  • Seven to fourteen days after surgery dressings are removed
  • Four to six weeks after surgery you can swim

Breast Augmentation and the Athlete: What Women Need to Know

Living an active lifestyle often means smaller breasts. Physical activity reduces body fat, and women don’t get to choose which areas lose fat. The belly and the breasts are both affected equally by fat reduction. Breasts are largely comprised of fatty tissue and lowered fat levels means smaller breasts. Many female athletes turn to breast augmentation to increase cup size and create a full, shapely breast appearance.

Breast augmentation is a highly effective option for increasing the breast size of female athletes. However, an active lifestyle does create a few complications for successful breast augmentation surgery. Breast Implant Size and Athletes

Ask any woman with large breasts and she’ll tell you that exercise isn’t always easy. A recent study found a direct correlation between breast size and activity levels. The larger a woman’s breasts were, the less likely she was to exercise. This is understandable. Breasts can make exercise difficult, especially high movement exercises like running.

If exercise and an active lifestyle are important to you, we’ll need to choose breast implant size carefully. Over or Under the Pectoral Muscle?

How Quickly Can I Return to Exercise After Breast Augmentation?

Breast Implant Type

Breast implants are available in three different types: saline, silicone gel, and form-stable (gummy bear) implants. Dr. Sajjadian uses all three types for natural looking breast augmentation. Due to safety concerns and the possible risks involved, Dr. Sajjadian does not use textured breast implants. Our smooth silicone gel implants have proven to be safe with no known risks using data from a 10-year study on implants.

  • Saline Breast Implants – Saline implants offer the unique advantage of unfilled placement. The silicone shell of the implant is placed into the breast unfilled. Once it is in place, the implant is carefully filled with a sterile saline solution. This allows for a smaller insertion incision. Saline breast implants are also FDA approved for use on younger women, 18+ compared to 22+ for silicone options. Saline breast implants are generally the most affordable breast implant option, averaging about 25% less than similarly sized silicone options.
  • Silicone Gel Implants – Silicone gel implants are the most popular breast implant option. The soft, smooth gel feels very much like natural breast tissue. These implants offer a softer feel than a saline implant, especially in women with less natural breast tissue for coverage. Modern silicone implants are sturdy, long lasting, and designed to last for many years.
  • Cohesive Gel Implants – The newest type of breast implant available is best known by its nickname, the “gummy bear implant.” These implants are a type of form stable breast implant, created using a thick, cohesive silicone gel shell. The durable shell of these implants offers superior shape retention and protection against leakage and ruptures. Cohesive gel implants are a popular option for natural looking breast augmentation as they have a tapered shape, thinner at the top with more projection at the bottom, mimicking natural breast shape.

Breast Implant Shape

Implants come in both round and shaped options. The shape of the implant helps to create the final shape of the breast. Both options can create a natural looking result when used properly and by a skilled plastic surgeon.

  • Round- Outside of the breast, a round breast implant looks similar in shape to a jelly donut. They are perfectly symmetrical and round, with a flattened middle. Round implants are available in both saline and silicone implant types. Round is the most common implant shape and creates a very natural looking result, both when standing and lying down. The saline or silicone inside of a round implant settles when standing, creating a natural breast shape that is wider at the bottom, thinner at the top.
  • Shaped or Anatomical Implants- Shaped breast implants are modeled after the natural breast shape. They are flatter and thinner at the top, with a fuller bottom. Shaped implants are a popular choice for the natural looking breast shape they create. Implant rotation is a risk with a shaped implant, but using textured implants and special implantation techniques can reduce the odds of this occurring. Shaped implants are also called anatomical breast implants or teardrop implants.

Implant Size

Breast implants are generally measured in CCs, a form of measurement that is very common in medical settings, but unfamiliar to many of our patients. CC is short for cubic centimeter and is a measurement of volume. If you’re more familiar with ounces, you can convert CCs to ounces by dividing by 29.5.

We recommend that you forget about CCs and cup size completely. Focus instead on the look you hope to achieve from your cosmetic breast surgery. Look at pictures of celebrities and friends, bringing us photos of your ideal breast size. The rice test can also be helpful. Fill the boot section of a pair of pantyhose with measured rice. Place in a sports bra and take your new look for a test drive. Add or subtract rice until you find your desired size. Take notes throughout the testing process, and bring them along to your consultation.

Size matters, but numbers don’t. We’ll help you create a look you love with appropriately size breast implants for a natural look.


As you prepare for your breast augmentation, we bet you’ll start noticing breasts. Pay attention to the many shapes and sizes of female breasts. Two women with identically sized breasts can have a dramatically different breast appearance, largely due to projection, or how far the breasts extend from the chest wall.

Think for a minute about an 8 oz. cup of water. You could pour the water into a short, wide vessel or a tall and thin one. The quantity of water doesn’t change, but its appearance certainly does. The same is true for implant projection. By modifying the size of the base of the implant, and how far the implant will extend off of the chest wall, we can create your ideal breast shape.

We offer low, moderate, and high projection implants, in every type, size, and shape of the breast implant we offer. Generally, high projection implants offer the fullest, roundest, and most noticeable results. Our natural looking breast augmentation patients generally prefer a moderate projection implant. Some women with a wide chest will benefit from a low profile implant.

Breast Implant Placement

Breast implants are placed inside of the breast, either over or under the pectoral muscle. Generally, a submuscular placement offers a more natural looking result. The muscle helps to cover and hide the implant, smoothing edges and reducing the odds of implant rippling.

Subglandular implant placement, or placement over the pectoral muscle, may yield a more natural looking result in patients that frequently lift heavy weights. With sufficient natural breast tissue, good results are possible with both placement options.

Breast implants are inserted into the breast through a small, carefully hidden incision. There are four primary options for incision placement. Dr. Sajjadian is experienced using all of the major breast implant incision options.

  • Peri-areolar- The peri-areolar incision is made around the outer edge of the areola, along with the line where darker areola and breast skin meet. Scarring fades to invisibility along the color transition, and most patients notice minimal scarring after they’ve healed.
  • Trans-axillary- The trans-axillary incision is placed under the arm, in the armpit. It leaves no breast scarring and a small incision that is unseen unless the arms are lifted. The trans-axillary incision can be used only once, so if revision surgery is needed, a new incision site must be selected.
  • Inframammary- The inframammary incision is the most popular breast augmentation incision by far. It is placed under the breast, in the fold where the breast meets the chest wall. This incision provides optimal access to the breast and muscle tissues for a very natural looking breast augmentation result. After surgery, the incision is hidden by the overhang of the breast and is practically unnoticeable on most patients.
  • TUBA- The TUBA technique for breast augmentation is very delicate and complex. Many surgeons do not have the experience necessary to offer this specialized and advanced technique. Dr. Sajjadian is one of only a few Southern California plastic surgeons to offer TUBA breast augmentation. TUBA is short for trans-umbilical breast augmentation. The incision is placed in the belly button, and an unfilled saline implant travels up from the abdomen into the breast, guided by an endoscope. Once it is in place, the implant is filled. This option leaves no breast scarring but is very time-consuming.

Complementary Procedures: Breast Lift for Sagging Breasts

Women with sagging breasts may benefit from a breast lift, performed at the same time as their breast augmentation. By combining these two procedures, we can treat moderate and severe breast sagging, while simultaneously enhancing breast size. Breast augmentation + breast lift is often performed after pregnancy, as part of our mommy makeover.

If sagging is present, a lift helps to ensure a good result from breast implants. Although an implant does eliminate a little sag by adding more volume, it doesn’t tighten skin. This leads to a saggy, deflated breast, even with implants. Combining a breast lift with implants ensures a good aesthetic result.

These two procedures are frequently combined and do not increase risk or recovery times significantly.

Material- Saline or Silicone?

Modern-day breast implants typically fall into one of two categories: saline or silicone. Dr. Sajjadian uses both of these breast implant types to craft stunning breast augmentation results for his patients. Breast implants are used to increase breast size, to enhance breast shape and fullness, to correct asymmetry, and to restore the breasts after pregnancy and breastfeeding. Both implant types can be used successfully for any of these purposes.

Saline Breast Implants

Saline breast implants feature a plastic outer shell that is filled with a medical-grade, sterilized salt water solution. Saline breast implants are FDA-approved for use on women that are 18+.

Saline implants offer several benefits including:

  • Lower Price Point- Saline implants are typically 25% less than similarly sized silicone implants.
  • Smaller Incision Size- Saline breast implants are placed into the breast unfilled. A deflated implant is much smaller than a filled one and incision size is considerably smaller with this implant type. This implant type also offers increased flexibility in incision types since it is placed into position unfilled.
  • Easy Rupture Detection- Saline implants deflate when they are ruptured. The saline solution is safely absorbed and expelled by the body. Saline is non-toxic.

Saline implants have a few downsides too. Some women find they feel a little too firm, not like natural breast tissue. This entirely depends on anatomy and preference. Saline implants are also more prone to wrinkling after extended use.

Silicone Breast Implants

There are two different types of silicone implants: silicone gel implants and form-stable implants, also known as gummy bear implants. Each type offers distinct benefits, so we’ll discuss them separately.

Silicone Gel Breast Implants

Silicone gel implants are our most popular option for breast augmentation. They are FDA-approved for use on women aged 22+. Our favorite benefits of these implants are:

  • A Soft and Natural Feel- Give your breast a quick squeeze. Now you know what a silicone implant feels like. These implants feel soft and natural, even outside of the breast.
  • Less Likely to Ripple- Silicone filled breast implants are less likely to ripple than their saline counterparts.

We recommend that patients with silicone implants receive regular checks to look for leaks for ruptures as the implant will not deflate if it is compromised. Another downside of silicone implants is the need for a larger incision and the increased risk of scarring.

Cohesive Silicone Implants

Gummy bear breast implants, also known as cohesive silicone implants or form stable implants, have a texture that is very similar to a gummy bear. Their advantages include:

  • Excellent Shape Holding- These breast implants hold their shape very well. They do not ripple.
  • Less Likely to Break- Form stable breast implants are filled with a thick, cohesive filling. They hold their shape, even if they rupture and are not likely to leak.
  • Natural Shape- The unique texture of these implants allows for a very natural implant shape. These implants are tapered, projected on the bottom and slimmed at the top. Many women love the beautiful breast shape that these implants create.

They are a firmer feeling implant, unnoticeable if they are well covered with natural breast tissue. Cohesive implants also require a larger incision with an increased risk of scarring.

There are four primary options for breast implant incisions: trans-axillary, peri-areolar, inframammary, and TUBA. The best incision type for you will depend on your anatomy, the size and type of breast implants that you intend to receive, and your goals for the procedure. Dr. Sajjadian understands that this is a complex choice and will carefully explain your options, along with offering his recommendations for your incision placement.

  • Trans-axillary- Trans-axillary breast augmentation incisions are placed in the armpit. This ensures no scarring on the breast. This incision can only be used one time, so if revision surgery is needed, a new incision will need to be made. Some patients dislike the visible scar that remains in the armpit after surgery.
  • Peri-areolar- The peri-areolar incision is made around the areola, right along the color transition. This hides scars and makes them practically unnoticeable for most women. The peri-areolar incision does limit the size, type, and shape of implant that can be used and may not be advisable for women with small areola or large implants. This incision provides good access to the breast for your plastic surgeon.
  • Inframammary- The inframammary incision is placed under the breast, in the crease where the breast meets the chest wall. It is well hidden by the overhang of the breast and is unnoticeable for most women after it has healed. This incision provides Dr. Sajjadian with the best possible access to the breast tissues. This is the most popular breast augmentation incision by far.
  • TUBA- The TUBA breast augmentation technique, also known as trans-umbilical breast augmentation, places the incision inside of the belly button. Using an endoscope, the breast implant is then carefully moved from the abdomen up into the breast area. Only saline implants are used with this technique. They are inserted deflated and filled after they are in position. The TUBA approach creates no breast scarring but is very complex and time-consuming. Only a few plastic surgeons in Southern California offer this specialized and advanced breast augmentation technique.

Dr. Sajjadian is very familiar and experienced using all four of these techniques. Many surgeons offer only one or two options for breast incision placement, but Dr. Sajjadian’s exemplary surgical skills allow him to provide his patients with more options and more treatment flexibility.


Breast implants can be placed over or under the pectoral muscle. Typically, a sub-muscular placement is preferred as it offers a more natural looking result, easier coverage of the implant, lower risk of bottoming out and capsular contracture, and better post-augmentation mammography results. Recovery tends to be longer and more difficult after this type of breast augmentation.

Some patients will be better served by a sub-glandular patient. This placement option typically offers a shorter, easier recovery and allows for a larger implant, provided there is sufficient breast tissue to cover it. Sub-glandular placement may also be preferable in patients involved in heavy exercise or weightlifting as the implant won’t distort when the pectoral muscle is flexed, as is common with a sub-muscular placement.

For both sub-glandular and sub-muscular placements, Dr. Sajjadian creates a pocket for the breast implant. The implant is placed in the pocket during breast surgery. At first, the pocket will be tight. In time, the skin and tissues surrounding the implant will relax, and the implant will settle into position. This is a process known as “drop and fluff” and takes 3-6 months to occur after surgery. This process usually happens quicker in sub-glandular placement patients.


There is no perfect size for the female breasts. We’ve seen women with stunning breasts at every cup size. Our goal is to find a breast implant size that complements your aesthetic goals and works with your anatomy.

When choosing an implant size, we are somewhat limited by the patient’s natural anatomy. An implant must be fully covered by breast tissue for a natural look. If we use an implant that is too large, ridges or bumps may be visible at the edges of the implant. Larger implant sizes may also be difficult, or uncomfortable, for smaller patients.

Dr. Sajjadian realizes that size is only one variable in the appearance of your breasts after augmentation. By varying implant shape, projection, placement, and other factors, he can create your ideal breasts, even if there are limitations to the size of implant your body can accommodate.

How Do You Find Your Ideal Implant Size?

Before your breast augmentation consultation, spend a little time contemplating your ideal breasts. Forget about cup sizes; letters and numbers matter less than appearance. Many patients also use an easy at home test to “try on” different breast sizes. It is called the rice test. Simply cut off and fill the boot section of a pair of pantyhose with rice. Place these rice sizers in a sports bra and try on all of your favorite clothing styles. Adjust the quantities of rice and take notes so you can share your research at your consultation with Dr. Sajjadian.
