What is Preservation Rhinoplasty?

Preservation rhinoplasty is a nose-adjustment surgery aiming to preserve much of the patient’s nose structural tissue.

The procedure requires sophisticated surgical skills. Dr. Sajjadian alters the nose’s shape by strategically eliminating small amounts of cartilage and bone from beneath the nose bridge. By doing so, he can reduce the height of the nose bridge (nasal dorsum) without shifting the patient’s dorsal lines.

Traditional destructive rhinoplasty techniques usually include removing the nasal hump by multiple floating nasal bone fractures and excision. These methods can create complications later during the natural healing process.

Preservation Rhinoplasty vs. Traditional Rhinoplasty

Most of our patients choose rhinoplasty to decrease the height of the nose bridge. For many, having an arch or hump in the bridge is undesirable.

Many patients experience a reduction in their dorsal hump height after traditional rhinoplasty but feel the results are unauthentic. This is primarily because of the alteration of the dorsal lines.

Because traditional rhinoplasty entails removing cartilage and bone from the top of the nose bridge, the operation significantly alters the natural bone structure of the nose. Thus, while traditional rhinoplasty does correct the problem of a large bump on the nose bridge, the procedure visibly modifies the nose’s natural lines.

Dr. Sajjadian alters the nasal bridge’s height with preservation rhinoplasty by taking cartilage and bone from underneath the nose. This technique maintains the natural appearance of the nose while adjusting the size of the bridge.

The benefits gained from preservation rhinoplasty are much more satisfactory for patients, and the need for revision rhinoplasty is less likely.

Preservation Rhinoplasty refers to three components:

  1. Raising the skin sleeve in a surgical plane known as the subperichondrial-subperiosteal plane
  2. Preserving the nose bridge (the cartilage and bone dorsum)
  3. Maintaining the top cartilages (alar) with minimal excision to achieve optimal shape with sutures.

For many years, surgeons preferred the sub-SMAS plane to elevate the skin sleeve because it is less disruptive than the previously employed subcutaneous plane. This dissection produced significant swelling, prolonged scar remodeling, numbness, and induration.

Long-term soft tissue thinning envelope was a major concern after traditional rhinoplasty. The soft tissue envelope with preservation rhinoplasty resulted in near-normal sensation, minimal swelling, minimal scar remodeling, and less long-term thinning of the soft tissue envelope.

Today, many rhinoplasty surgeries involve resecting the dorsal hump to create an open roof requiring the breaking of cartilage and bone. Preservation rhinoplasty preserves dorsal hump structures while eliminating the hump through a septal resection. This modifies the bridge without destroying the nose’s anatomy.

Traditional rhinoplasty achieved tip shape correction with a combination of incisions, excisions, grafts, and sutures. Results are generally good at first, then a significant percentage of these cases will degrade over time.

The adoption of structural support through columellar struts, tip suturing, tongue and groove procedures, and septal extension grafts have dramatically improved intermediate results. There are fewer tip deformities and projection maintenance.

Patients who seek to correct a bump or reduce nose projection are possible candidates for a preservation rhinoplasty in Orange County. Those who have had a previous nose surgery are usually not good candidates for preservation rhinoplasty.

Expectations for Revision Rhinoplasty in Orange County

To prepare for a revision rhinoplasty procedure, the patient must understand what to expect before, during, and after the operation.


As you lead up to your preservation rhinoplasty, expect Dr. Sajjadian to prescribe a healthy diet, plenty of rest, and to plan for your recovery.

If you smoke, stop for six weeks before the surgery, as smoking will hinder your recovery. Nicotine is a vasoconstrictor that inhibits blood circulation to the skin, impairing the body’s ability to heal.

Plan to bring a responsible adult with you to the operation. You will be under the influence of a general anesthetic during the surgery. Your ability to drive afterward will be impaired.


On the day of surgery, expect a final last-minute consultation with Dr. Sajjadian to clarify any questions you may have. When you’re ready, the anesthesiologist will administer your general anesthetic before Dr. Sajjadian starts the operation. The surgery generally takes between one to two hours. 


Once Dr. Sajjadian has completed the procedure, he will place splints on your nose, along with a protective gauze.

Your private nurse will monitor you as you wake up from the anesthetic. Once Dr. Sajjadian deems it’s safe for you to go home, your friend or family member can drive you back to recover at home. Expect recovery to last about six weeks.

Preservation Rhinoplasty FAQs

The six weeks following your preservation rhinoplasty are vital in ensuring a successful recovery.

Dr. Sajjadian will discuss what to do during these six weeks. He will see you after one week to ensure your recovery is going smoothly.  

You will need complete rest during the first week of recovery, which is best to spend at home. A nasal drip pad will be utilized for the first two days to handle any bleeding. You will also need to irrigate your nose after surgery by using nasal saline. You will continue this for three months.

Do not take your recovery lightly. Dr. Sajjadian will provide detailed instructions on the dos and don’ts during this time. If followed, you can expect a successful healing process and outcome as well.

Most patients return to work about 10 days after the operation. This will, of course, depend on the type of work you do. This will be discussed further during your consultation and subsequent visits.

Patients may resume their regular workout routine three to four weeks after the operation. You’ll need to limit physical activity for two days. After that, you may start moving around the house and doing light workouts.

After one week you can start walking in the neighborhood. Of course, this depends on your recovery progress. It is best to err on the side of caution. It would be best if you gave your body time to heal according to what it tells you. Please don’t push it.

Adequate sleep and resting will not only help you achieve the best results but minimizes the chance of any potential complications.
