Position Breast Implants: Under or Over?

The question of whether to put breast implants ‘under’ or ‘over’ the pectoralis major muscle is addressed frequently on websites that talk about breast implants.  Whether the breast implants are placed under or over the muscle depends primarily on which option gives the most natural appearance and desired results.  However, in addition to deciding where to place the breast implants in relation to the muscle, consideration also needs to involve where they are placed vertically and horizontally on the chest.  Several of our clients in the Newport Beach area also need the infra-mammary fold (that crease below your breast) lowered in order to allow their breast implants to rest at a level that appears natural relative to the position of the nipple and areola. This also prevents the appearance of excessive fullness in the upper portion of the breast compared to the part below the areola.

If you turn sideways and look at your natural breasts at a profile, the natural-appearing breast is not convex (curved like the exterior of a circle) in the top portion. An excessively curved and overly full upper portion of the breast is a dead giveaway that the breast implant sits right below the skin. Likewise, if the infra-mammary fold is lowered too far, the enhanced breast will appear ‘bottomed out’, with a nipple/areola that appears to sit too high on the breast. This is another situation with a distinctly unnatural appearance and when possible should be avoided.

Great attention should also be placed on deciding the horizontal position of breast implants. Breast implant pockets that extend too much to the side will result in breasts that have an excessively wide space between them in the cleavage area. The breast implants will also appear abnormally far apart. If the other extreme occurs and the pockets do not extend far enough laterally the result is a breast augmentation with an abnormal “pushed together” appearance. To get that coveted ‘hourglass figure’ that many of our Newport Beach clients desire, we place careful attention to ensure that all aspects of their new breast implants are appropriate and natural looking.
